This page offers students, faculty, staff, and community members a convenient way to report concerns, incidents, behaviors, or observations to university administrators. Undergraduate Success Alert
Any concern and/or behavior that could impact the academic success of an undergraduate student. Examples include missed class, lack of engagement, difficulty with course material, tutoring referral, lack of basic needs, etc. (Faculty & Staff)
Graduate Success Alert
Any concern and/or behavior that could impact the academic success of a graduate student. Examples include missed class, lack of engagement, difficulty with course material, lack of basic needs, etc.
Behavior/Wellness Alert
Any concerns for a student's overall wellness: mental health concerns; aggression or threats; suicide ideation or attempt; substance abuse; noticeable changes in behavior or appearance, etc. If the student of concern you are referring poses an immediate threat to self or others, please call 988 or 911.
General University Complaint
General University complaints that do not fit into another form category on this page. Formal complaints may include (but are not limited to) issues regarding classroom instruction, college policies, procedures, services and office.
Discrimination/Harassment
Sexual Misconduct Report
This form can be used by any member of the Emporia State University community to report a concern of discrimination or harassment, sexual misconduct, stalking, or dating or domestic violence in which anyone involved is a member of the Emporia State University Community. Click here to review ESU's Title IX Policy, Process, or ESU's general discrimination and harassment Policy. Filing this incident report constitutes official notice to the Emporia State University. The information will be used to initiate the offering of supportive services as explained in the Title IX Sexual Harassment, Discrimination and Retaliation Policy and to investigate the matter.
Hazing Report
Any behavior related to hazing that involves anyone who is involved with the Emporia State University campus community.
Anonymous Reporting Hotline
The hotline is an anonymous, 24/7 resource for reporting concerns regarding legal or policy violations or unethical behavior. Employees who believe they are aware of a violation are expected and encouraged to report their concerns. Various reporting methods are available for reporting an incident.
