Open Enrollment Elections:

To make your insurance elections for 2018, follow these instructions:

  1. Log into the MAP system (https://sso.cobraguard.net/seer_login.php)  
    1. Select Emporia State University in the drop down box
    2. Enter in the same User ID and password you use to get into Buzz-In
      1. If you need assistance with your ID and password, contact the help-desk at 341-5555
  2. Click on the Enrollments and Events tab
  3. Click Launch Enrollment (you should see one for Initial Enrollment for 2017, and another for Open Enrollment for 2018)
  4. Click on Change Your Elections
  5. Follow the prompts to complete your enrollment
  6. On the last page, scroll to the very bottom of the page and click the green Save and Print Confirmation button
  7. In the pop-up window, again, click on the green Save and Print Confirmation button (yep, that’s twice!)
  8. When it returns you to the final screen, scroll to the bottom and you can either print or save your elections, or you can return to your Account Overview.
    1. A copy should also be sent to your email address.
    2. Please be sure you have a copy of your enrollment

 

Descriptions of the insurance options are listed below, including changes for the 2018 Plan Year. Join representatives from the State Employee Health Plan, as they lead a group presentation about the details and changes to the state's benefits for the 2018 calendar year. Summary of Changes for 2018

Plan Options

  • Added three new plans (Q, N, and J)
  • There is no difference between premiums for Aetna vs BCBS this year
  • New benefit added to medical coverage: Amwell/Telehealth with BCBS, or Teledoc with Aetna
  • New vendor: Met Life is offering optional coverage for: Accidental Injury, Hospital Indemnity, and Critical Illness
    • Replaces Colonial Life- employees can work with Colonial Life to maintain benefit
  • Castlight will no longer be provided in 2018

 

 

Health Insurance

Plan Options

Plans A and Q are traditional PPO plans, and Plans C and N are qualified high deductible plans.  Plan J was designed to meet the requirements for employees on a J-Visa (but anyone can elect plan J).  All options cover 100% of the network provider costs of preventative care services including: a well-person exam, immunizations, routine hearing exam, routine vision exam, and other age-appropriate preventative services.  The chart below offers a quick comparison between the plans while using Network Providers, and a more in-depth comparison is available on page 16 of the SEHP Open Enrollment Book.

Cost of Premiums

Employees and spouses can each earn an additional $500 employer contribution by participating in the HealthQuest Rewards Program (see details in the HealthQuest section below).

 

HealthQuest Rewards

In 2018, employees and spouses can participate in the HealthQuest Rewards Program to each earn a $10 discount on the semi-monthly health insurance premiums for 2019.  Employees and spouses covered by Plans C, Q, N, and J in 2018 are also each eligible to gain an additional $500 employer contribution into their HSA (Plan C or N) or HRA (Plan Q or J) during 2018.

In order to receive the discounts and incentives, the participant must use their email address to log-in to the HealthQuest Portal, and complete the Health Assessment Questionnaire. If it is your first time logging in, you will need to create a new account (instructions here). The participant must earn at least 40 points to receive the 2019 premium discount, and 50 points to receive the entire HSA/HRA contribution.  Points are earned by completing and recording any of a variety of health related behaviors including: well-person exam, dental exams, vision exam, tobacco cessation program, condition management sessions, and other online or telephone activities.  In general, all of the activities are covered 100% by the health plan, or are otherwise free to the employee.

Healthy Kids

The Healthy Kids program helps eligible employees cover the cost of the premiums for their children enrolled in the State Employee Health Plan.

For families with children enrolled in the Healthy Kids program, the 2018 semi-monthly premium of each plan is described in the chart below.  The premiums do not change regardless of how many children are covered.

NEW Amwell/Telehealth and Teledoc

In 2018, BCBS is offering Amwell/Telehealth, and Aetna is offering Teledoc.  Both of these services allow plan members to electronically (phone, tablet, computer) meet with a doctor for common conditions such as cold, flu, fever, rash, pink eye, ear infection, and migraines.

For the BCBS Amwell program

  • Plan A is a copay and would be $10.  Just like other copayments made it will apply to the out of pocket maximum.  You would not have to satisfy the deductible. 
  • Plan C is a $49 fee.  This $49 is applying to your deductible and once you satisfy your deductible it will apply the appropriate 20% coinsurance until out of pocket is met.

For the Aetna Teladoc program

  • Plan A is a copay and would be $10.  Just like other copayments made it will apply to the out of pocket maximum.  You would not have to satisfy the deductible.

Dental Insurance

Each person must be enrolled in health insurance to be eligible for dental insurance, provided by Delta Dental.  Anyone enrolled in health insurance can opt out of dental coverage.  Those electing dental coverage will pay an additional semi-monthly premium as outlined below.

Basic Vision Plan Coverage
A $25 material copay to lenses (also applies to frame purchases but not contacts), then the policy covers:

  • 100% on single-vision, standard bifocal, trifocal, or lenticular lenses.
  • Up to $100 retail allowance for frames
  • Elective contact lens allowance of up to $150 (must be used all at once)
  • Home delivery of contacts at: contactsdirect.com
  • Allowances available now for Polycarbonate, Scratch and UV coating.
  • Contact lens fitting fee (with a $35 copay for standard contact lenses)

Enhanced Vision Plan Coverage
The Enhanced vision plan includes all Basic plan coverage, along with:

  • Standard progressive lenses covered up to $165
  • High-index lenses or poly-carbonate lenses covered up to $116
  • Polycarbonate, Scratch and UV coating covered in full
  • Frame allowance on Enhanced plan is now $150 retail allowance

Special Features from Surency Vision
Members may use their contact lens benefit AND frame allowance in the same plan year; however, eye glass lenses would be the member's responsibility because the lens allowance was utilized on contact lenses.

Additional Discounts with Network Providers are available for the below services (see the Basic or Enhanced Certificate of Insurance for full details).  These discounts are available from Network providers unless the provider directory states ““May not accept all additional plan discounts” in the provider directory. Please note additional discounts are not available from Non-Network providers.

  • 20% Discount for lens add-ons or upgrades not otherwise covered by the plan.
  • 20% Discount for frames costing above the plan allowance.
  • 40% Discount on additional pairs of glasses (unlimited).
