Procedure/Fee Schedule, Fall 2005 - Spring 2006

ESU Health Services is available only to degree-seeking, currently enrolled ESU Students who pay campus privilege fees:
Please call 620-341-5223 to schedule your appointment.  Students who become acutely ill during the school day may walk-in, but may have a longer wait as they must be worked into the regularly scheduled appointments. Our busiest times are around lunchtime.

If you make a 30" appointment for a pap smear or extended visit and do not show up or call to cancel, there will be an $11 charge. There is also an $11 charge if you miss more than two appointments without calling to cancel.  If you are late to your appointment, you may be asked to reschedule.

Please fill out a Health History Form either at the office, before your visit, or download it from this site. To download the form, you will need Adobe® Acrobat® Reader. (Click here for a free copy of Adobe® Acrobat® Reader.) There are two pages to the form, so make sure you download and print out both pages. (To get back to this page once Adobe® Acrobat® Reader is open, use your browser's "back" button.)

Your visit is covered by the student health fees you paid at registration. Other services require a small fee.  Click here for list of fees.

We do not accept insurance assignments, except through the State of Kansas student plan, United Healthcare-Student Resources. If you are interested in buying this optional plan, we can give you a brochure. If you have this student insurance, please let us know when you first check in with the Health Services receptionist. For insurance other than the United Healthcare-Student Resources plan, we will give you a paid receipt. You may send this in to your (or your parents') private insurance company if you choose.
You are responsible for any charges incurred at the time of service. We accept cash, credit cards or checks.
