These are the forms that we commonly use in the Kansas Small Business Development Center in Adobe PDF format. Feel free to print these out, fill them in and then bring them to our office.
Request for CounselingTo become an KSBDC client and receive FREE confidential consulting, you will need to complete each of these forms. You may fill in the forms and submit electronically or print and submit via mail. Someone with the KSBDC will contact you shortly after receipt of the forms to gather more information and/or to set up an appointment.
