Second Bachelor Application for returning ESU Students*

This on-line application is to be completed for RETURNING ESU STUDENTS ONLY.  *NEW TO ESU STUDENTS: Apply through Admission at http://www.emporia.edu/admiss
If you cannot submit your application on-line, please download the application and mail along with your application fee. Please read application instructions before completing and submitting this form. A $30 application fee and a $10 processing fee (total $40) must be paid if you were not previously admitted to Emporia State University as a degree-seeking undergraduate student.

You will be required to print the signature block on the last part of this application. If a printer is not attached to your computer then we recommend that you save the signature block to a file, copy it to a floppy disk and print it at a computer that has a printer available.

Any Information you submit is insecure and could be observed by a third party. If you are submitting information you would like kept private, it would be safer to cancel the submission

Note: If you are beginning work toward a second degree, one transcript from each institution attended must be sent directly from the institution to Emporia State University, Degree Analysis Office, 1200 Commercial Campus Box 4026, Emporia, KS, 66801. Admission cannot be acted upon until transcripts are received. (Junior college transcripts are required if grades are not included on Bachelor's degree transcript.)
