Online Application Forms

Application for Undergraduate Admission - This application is to be filled out by degree-seeking undergraduates who have never attended ESU as a degree-seeking student. A $40 application/processing fee is required.

Undergraduate Readmission Application - A readmission application needs to be filled out and returned to the Registration Office if an undergraduate is returning to ESU after being absent for one semester or more. Since you have paid the initial application fee to the Admission's Office, there is no fee for being readmitted.

Application for Admission to Graduate Study - This application is to be filled out by students seeking admission to graduate study. A non-refundable $40 graduate application fee is required for all graduate students.

Graduate Readmission Application - This needs to be completed by returning students in graduate study who have not taken courses for at least a year. No fee is required.

Tuition & Fee Forms

Application for Resident Fee Privilege (Out-of-State Residents Employed in Kansas) - This application is to be filled out by Out-of-State Residents who are employed in Kansas in order to obtain residency rates for Lifelong Learning courses. This form must be completed each semester of enrollment. The waiver is not applicable for on-campus courses.

Application for Resident Fee Privilege (Recruited or Transfered Employees to Kansas) - This application is to be filled out by students who have been recruited or transfered to work in Kansas in order to obtain residency rates for Lifelong Learning courses. This form must be completed each semester of enrollment. The waiver is not applicable for on-campus courses.

Application for Resident Classification for Fee Purposes - Use this form to apply for residency status. Visit the Registration Website for details and instructions.

Remittance Form - To be completed and submitted with tuition payments.

Installment Payment Plan (Fall 2007) - Under the Installment Plan, the initial payment will be 1/3 of the amount due plus the $25.00 administrative fee. The balance is to be paid in two additional installments.

Tuition and Fee Deferment (Fall 2007) - Under the Tuition and Fee Deferment, the full payment is due by the deferment date granted.

Direct Deposit Authorization - Federal student financial aid can only be direct deposited into a student’s account by obtaining written authorization to deposit these funds. This form will allow Cashiering Services to direct deposit any refunds or excess financial aid to your Lyon County State Bank checking account.

Title IV Authorization - The only way federal student aid funds can be used to pay for “non-institutional charges” is to obtain your written authorization to pay those charges.  This form will allow Cashiering Services to apply your federal financial aid to your non-institutional charges.
