1. Please supply as much information (such as Author, Title, Publication information and ISBN No.) as possible concerning the library materials you are recommending for purchase.


2. Please provide your name, email address or mailing address, and phone number, in order for us to contact you for further information or notification.


3. Iif known, indicate catalog or publication information

4. This service is primarily for Emporia State University faculty, staff, and students.
