Intent To Graduate

The following form must be completed the semester preceding the semester you anticipate completing degree requirements. Due March 1 for Summer graduates, July 1 for Fall graduates, and November 1 for Spring graduates. Failure to give notice of intent to graduate will cause a delay in the awarding of your degree. If your browser does not support forms, please contact us by email, fax, letter or phone and we will mail you a form.

This application is valid for three terms only. Failure to graduate after the third term REQUIRES the cancellation of this application and the submission of a new one.
