Documentation

Documentation of each disability must be submitted to the office in support of any accommodations and services that are being requested. Students are responsible for submitting documentation of their disabilities to the office. We can assist students by faxing documentation requirements to evaluators, if requested. Documentation requirements are available after the initial meeting with the director or by contacting the director in advance of enrolling. Documentation needs will vary by the type of disability and severity of the impact on the student’s current functioning in the educational setting. Depending upon the variability of the disability and the medication needs of the individual, updated documentation may be required by the director so that the current impact can be determined. In the event the condition changes or treatment plans are modified, students are well served by having updated documentation submitted to the office so that accommodations and services can be adjusted based upon individual needs.

When submitting documentation, original documents need to be submitted unless the information is being faxed directly from the evaluator. Evaluators will need the student's consent in order to fax information directly to our office. When students request documentation requirements be faxed to an evaluator, they are served well by being prepared to share the name of the evaluator and the respective fax number.

After documentation is submitted to the office, students can schedule an appointment with the director to discuss individual needs. We recommend that students start the documentation process as early as possible so that a smooth transition to Emporia State can take place.
