• Starting and Using a Credential File

This is your guide for starting and using your credential file!!!

• Consent to Disclose Information

This form is filled out by student/alumni to start your credential file and is to be turned into career services with the $15 annual fee for "Full Access" to our new online system.

• Reference Form to give to Writer

The student/alumni fills out the upper right corner of form, then the writer fills out the upper left corner of the form and returns it to career services with their letter of reference. This form must accompany the letter of reference or the letter will be considered an "Open" letter.

• Credentials Request Form (Electronic Form)

Don't have time to visit us? Go online and request your credentials be sent out and pay for them from wherever you are!

• Credentials Request Form (Printable Version)

Use this to request your credential file be sent out. Drop off, fax or mail to Career Services, 433 S Morse Hall.

• Authorization to Remove Letters of Reference

We must have your authorization and signature to "hold" or "destroy" your old letters of reference.
