Graduate Degree Forms
These are the different forms that you need to complete to be admitted to degree candidacy. You only need the one degree plan for your program.

All of the following forms are formatted in Microsoft WORD for Windows. Hold the shift key down and use your mouse to click on DEGREE PLAN to download the form for your program and PREVIOUS COURSEWORK, which is copied onto the back of the degree plan. All students will also need the COVER SHEET for the research proposal. Instructions for downloading and saving the file will be displayed. If you have trouble receiving the files or the files are not in a usuable format, please contact the Graduate Coordinator, Scott S. Crupper, at scrupper@emporia.edu.
