IMPORTANT: This form is NOT required for: a) for students seeking degrees in Teacher Education who achieve passing scores on the reading, writing, and mathematics subtests of the Pre-Professional Skills Test (PPST); or Collegiate Assessment of Academic Proficiency (CAAP) test. (This form is necessary ONLY if the student is seeking a degree in Teacher Education, AND qualifies for the Window option); or b) for other students who fulfill the requirement by achieving passing scores on the PPST or the (CAAP) test.
